Resolution:
BE IT RESOLVED, that Agreement A176878 with the Minnesota Department of Human Services, accepting a grant to fund services related to the Transitions to Community Initiative for the period January 1, 2017 through December 31, 2017, in the receivable amount of $651,000, be approved; and

 

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:

Agreement A176878 accepts grant funding from the Minnesota Department of Human Services Mental Health Division to improve the transition from the hospital to a community placement for patients at Anoka Regional Treatment Center (AMRTC) and Minnesota Security Hospital – St. Peter (MSH) who have complex mental health treatment and other service needs.

 

Transitions funds will be used to better serve patients at AMRTC and MSH with a mental illness who are experiencing a delay in their discharge to the community due to difficulty of placement or other reasons. The grant will focus on individual choice in discharge planning, while moving individuals through the discharge process in a more timely manner. The grant will also identify and address system gaps to create long-term system solutions to typical barriers to discharge.

 

This grant funding is for the year 2017 in the amount of $651,000.
