Agmt A2010337 with MN Dept of Human Services, to provide innovative mental health services, 07/01/20-06/30/21, $215,457 (recv), add 1.0 grant-funded FTE
Resolution:

BE IT RESOLVED, that Agreement A2010337 with the Minnesota Department of Human Services to provide community-based outpatient mental health services and to reduce admissions to regional treatment centers and community behavioral health hospitals, during the period July 1, 2020 through June 30, 2021 in the receivable amount of $215,457 be approved; that the Chair of the Board be authorized to sign the agreement on behalf of the county; and that the Controller be authorized to accept and disburse funds as directed; and

 

BE IT FURTHER RESOLVED, that a supplemental appropriation of 1 grant funded FTE be added to the 2020 Human Services and Public Health budget; and

 

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:

Minnesota Department of Human Services awarded Hennepin County funds in response to a competitive proposal. The grant agreement A2010337 will be used for integrated mental health services provided at the Behavioral Health Care Center at 1800 Chicago Avenue in Minneapolis. This is the second biennial grant for services.

 

The first Innovations grant agreement A188719 for $867,074 runs 6/25/2018-9/30/2020 (last amended through Resolution 20-0198 on 5/19/2020). The grant project has had 747 client visits from 9/1/2018- 8/17/2020. In that time project counselors completed 681 referrals to community mental health, 381 referrals to economic support services and 483 referrals to housing. Services will continue to target approximately 1,000 Hennepin County residents with serious mental illness and complex co-occurring conditions who cycle from detention to intensive psychiatric hospital services, to the street, and back. The goal is to divert this population into innovative, collaborative, and community- based mental health services.

 

This grant agreement will fund a new 1.0 FTE Senior Social Worker embedded at the county jail. Funds will also support a part time nurse practitioner and contractors including Mental Health Resources and Avivo and Hennepin HealthCare Services. A2010337 is funded at $215,457 for the period July 1, 2020-June 30, 2021. The Department anticipates that the Minnesota Department of Human Services will extend the grant agreement to a new end date of 6/30/2022 with additional funds of $317,105 for a total grant award of $532,562.00 for 24 months.
