BE IT RESOLVED, that Amendment 3 to Agreement PR00001069 with Headway Emotional Health Services amending the agreement to update the case fee paid to the Minnesota Department of Human Services for Child Welfare-Targeted Case Management cases from $400 per case to $427 and extending the contract end date through September 30, 2022 be approved; that the Chair of the Board be authorized to sign the Amendment on behalf of the County; and that the Controller be authorized to disburse funds as directed. |

The Be@School program focuses on addressing barriers to school attendance. Hennepin County is committed to equitable outcomes across racial and cultural groups. This board action request aligns with the Hennepin County disparity reduction efforts by supporting educational opportunities for all students residing in Hennepin County. The target population is students between ages 5-17. Headway Emotional Health Services provides service to children at risk for truancy or educational neglect. The new rate change was determined and set by the Minnesota Department of Human Services for child welfare-targeted case management services and is effective July 1, 2022. | |
