Resolution:

BE IT RESOLVED, that Agreement MDHA188923 with the Minnesota Department of Health for costs associated with developing a new home visiting model during the period May 1, 2018 through December 31, 2022 in the receivable amount of $2,086,623, be approved; that the Chair of the Board be authorized to sign the agreement on behalf of Hennepin County, and that the Controller be authorized to accept and disburse funds as directed, and,

BE IT FURTHER RESOLVED, that the Hennepin County Public Health Department be authorized to receive a supplemental appropriation of $428,212 to the 2018 Hennepin County Public Health Department budget, and,

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by the county for this program when grant funds are no longer available.

Background:

Agreement MDHA188923 is a new Evidence Based Home Visiting (EBHV) grant from the Minnesota Department of Health to provide evidence-based home visiting models that best serve families in need in Hennepin County. Primary goals of the grant include revising and streamlining current referral processes to maximize effectiveness and direct families to the best model for their needs; optimizing program outcomes and identifying best practices; and testing new approaches in home visiting to increase efficiency and effectiveness of the programs.

The total obligation of the State for all compensation and reimbursements to the Hennepin County under this grant project agreement will not exceed $2,086,623 as follows:
