BE IT RESOLVED, that Amendment 4 to Agreement MDHA152946 with the Minnesota Department of Health to educate and treat individuals diagnosed with infectious diseases for the grant period January 1, 2015 through December 31, 2019, increasing the receivable by $252,500 for a new total receivable amount of $944,500, be approved; that the Chair of the Board be authorized to sign the Amendment on behalf of the County; and that the Controller be authorized to accept and disperse funds as directed, and
BE IT FURTHER RESOLVED, that the Hennepin County Public Health Department be authorized to receive a supplemental appropriation of $79,070 to the 2018 Hennepin County Public Health Department budget, and
BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by the county for this program when grant funds are no longer available.

The Minnesota Department of Health (MDH) has a grant project agreement with Hennepin County Public Health to conduct a program to assure that individuals who are diagnosed with and/or treated for Syphilis, Gonorrhea, Chlamydia, and HIV infection within the Public Health Clinic receive treatment to cure the infection and prevent disease complications. Patients also receive counseling to help prevent the spread of the diseases.
Current Request: Amendment 4 to Agreement MDHA152946 increases the receivable amount by $252,500 and seeks approval of a supplemental appropriation of $79,070 to the 2018 Hennepin County Public Health budget.
