Resolution:

BE IT RESOLVED, that Agreement A189250 with Minnesota Department of Human Services for the provision of a System of Care to provide services and supports to youth who have or are at risk of mental health challenges during the period July 1, 2018 through December 31, 2021, in the receivable amount of $561,900, be approved; that the Chair of the Board be authorized to sign the agreement on behalf of the county; and that the Controller be authorized to accept and disburse funds as directed; and 

 

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for the program by Hennepin County Board of Commissioners does not imply a continued commitment by Hennepin County for this program when grant funds are no longer available.

Background:

Hennepin County Human Services is seeking to develop a System of Care (SoC) that has a coordinated spectrum of community based services and supports for children and youth who have or are at risk for mental health and related challenges for their family. This SoC will build meaningful partnerships with families and youth, address cultural and linguistic needs, and focus on how youth can function better at home, in school and in their community. The goal is to make system changes to decrease the use of congregate care and reinvest those dollars in evidence-based, community-based services to support youth to maintain community living. 

The grant was awarded in the amount of $561,900 to be paid out over 3 years; payments will be received annually in the amount of $187,300 for each year (2018-2019, 2019-2020, 2020-2021).

This grant's focus is children that have complex mental health needs and are involved in multiple child-serving systems. Funds will be used to implement a pilot program from the National Wraparound Implementation Center (NWIC) comprehensive care coordination model. This pilot project will be a program that will support our larger array of Children’s Mental Health Services and align with SoC values.
