BE IT FURTHER RESOLVED that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of  Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:

Agreement A189377 is a two-year grant from the Minnesota Department of Human Services Mental Health Division to improve the transition from the hospital to a community placement for patients at Anoka Regional Treatment Center (AMRTC) and Minnesota Security Hospital – St. Peter (MSH) who have complex mental health treatment and other service needs. Minnesota Department of Human Services has provided Transition to Community Initiative Grant Funds for a number of years, most recently in Agreement A178548.

Transitions funds will be used to better serve patients at AMRTC and MSH with a mental illness who are experiencing a delay in their discharge to the community due to difficulty of placement or other reasons. The grant enables customized discharge planning, and allows for more timely discharges. Funds will help staff to identify and address system gaps to create solutions to typical barriers to discharge. This grant funding is for a total of $1,302,000.00, including $651,000 for the period January 1, 2019 through December 31, 2019. Another $651,000 is awarded for the second year January 1, 2020 through December 31, 2020.
