Resolution:

BE IT RESOLVED, that Agreement MDHA189316 with the Minnesota Department of Health accepting a grant for the Hennepin County Public Health Department for the period of September 30, 2018 through September 29, 2023, that the receivable amount of $225,901 awarded September 30, 2018 to September 29, 2019 be approved; that the Chair of the Board be authorized to sign the agreement on behalf of the County; and that the Controller be authorized to accept and disburse funds as directed; and

BE IT FURTHER RESOLVED, that the Hennepin County Public Health Department budget be authorized to increase their staff complement by a 1.5 Full Time Equivalent (FTE) and receive a supplemental appropriation of $225,901 to the 2019 Public Health Department budget, and

BE IT FURTHER RESOLVED, that acceptance of grant funding for the program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:

The Minnesota Department of Health (MDH) has been awarded funding from the Centers for Disease Control and Prevention (CDC) for RFA DP18-1817, Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke. The funding from CDC has a duration of five years, ending September 29, 2023. For the first year budget period of September 30, 2018 to September 29, 2019, $225,901 of the grant funds, comprised of $80,000 to support the Component A diabetes work and $145,901 to support the Component B cardiovascular disease work will be awarded to Hennepin County Public Health by MDH. Future year funding will be based on availability of funds at CDC’s level. MDH’s intent is to work with Hennepin County Public Health for the duration of the five-year period and to keep the funding level stable to allow for maintenance of project staff.

The MDH will work with the Public Health Promotion and Mental Health Clinic Programs within Hennepin County’s Public Health Department. Implementation plans include the following:

 

  • Support organizations in increasing enrollment in existing CDC-recognized lifestyle change programs or establishing and sustaining new CDC recognized lifestyle change programs in underserved areas;
  • Implement tailored communication/messaging to reach underserved populations at greatest risk for type 2 diabetes to increase awareness of prediabetes;
  • Increase adoption and use of clinical systems and care practices to improve health outcomes for people with diabetes;
  • Increase identification of patients with undiagnosed hypertension;
  • Explore and test innovative ways to engage non-physician team members in hypertension and cholesterol management in clinical and community settings;
  • Explore and test innovative ways to expand use of telehealth including mobile health technology to promote management of hypertension and high blood cholesterol;
  • Explore and test innovative ways to enhance referral, participation, and adherence in cardia rehabilitation programs in traditional and community settings, including home-based settings.
